I have been informed of the necessary requirements regarding veterinary care, vaccinations, <\/span>neuter\/spay, licensing, food, water and shelter. The Richland Area Rescue has my permission to <\/span>check any of these answers that I have provided and I am aware that any false information <\/span>provided will result in the immediate rejection of this application.<\/span><\/p>\n

If the animal has not been altered, I agree to have him\/her spayed\/neutered with in 30 days or by <\/span>6 months of age. I also will send proof of such alteration to: Judy Elliott, 30083 County <\/span>Highway O, Richland Center, WI 53581. 608-647-8697. If said alteration is not performed, <\/span>Richland Area Rescue has the right to reclaim this animal.<\/span><\/p>\n

ADOPTION FEE. <\/b>\u00a0I understand that this adoption fee is non-refundable.\u00a0 I also understand that I am responsible for expenses for the veterinary care, medication, food, supplies and all other expenses for the animal after the date of adoption.<\/span><\/p>\n

PURPOSE OF AGREEMENT.<\/strong> I understand that Richland Area Rescue, Inc uses Agreements to protect animals who come under its care. I also understand that if I am unable or unwilling to follow through on my commitment to the above-named animal, this Agreement gives Richland Area Rescue the mechanism for recovering said animal.<\/p>\n

RECEIPT.<\/strong> I hereby acknowledge receiving from the Richland Area Rescue the animal described above. I have been informed that this adoption is conditional so far that Richland Area Rescue requires me to continue to provide loving care for this animal and that if I fail to abide by any terms of this Agreement, Richland Area Rescue will reclaim said animal. I also understand that in order to insure compliance with this Agreement, Richland Area Rescue reserves the right to inquire as to the status of the animal\u2019s care.<\/p>\n

LONG-TERM CARE.<\/strong> I agree to provide regular and ongoing veterinary care for the animal described above. If the animal is a dog or cat, he or she will be spayed or neutered by six months of age (if not done at the time of adoption). I agree that I am responsible for the veterinary care for the animal described above from the time the animal leaves the custody of Richland Area Rescue.<\/p>\n

SURRENDER.<\/strong> If for any unforeseen reason I must give up this animal, I agree to return to or arrange for rehoming the animal through Richland Area Rescue. I will contact Judy Elliott at (608) 604-8213 to arrange for return of or assistance in rehoming the animal. I will not transfer this animal to another person without the prior approval of Richland Area Rescue.<\/p>\n

DISCLOSURE AND RELEASE:<\/strong> I understand that:
Animals are different from human beings in their responded to human action and may be unpredictable;
Animals should be closely supervised when they are with children or vulnerable adults;
An animal\u2019s behavior may change after he or she leaves his or her housing at the rescue and adjust to his or her new home; and
Although Richland Area Rescue has provided you with a personality and health profile of the animal identified above, said animal may display other and varied personality characteristics which may differ from the personality represented by Richland Area Rescue.<\/p>\n


This Agreement shall be binding and insure the benefit of Adopter and Richland Area Rescue and their respective heirs, legal representatives, successors, and assigns.<\/strong><\/p>\n

This Agreement shall be subject to and governed by the laws of the State of Wisconsin. The invalidity or unenforceability of any particular provision of this Agreement shall not affect the other provisions hereof and this Agreement shall be construed in all respects as if such invalid or unenforceable provision were omitted.<\/p>\n

I hereby accept responsibility for the animal identified above and hereby release and discharge Richland Area Rescue, Inc from liability for any injury or damages to any person or property caused in the future by said animal, and from any causes of action, claims, suits, or demands whatsoever that may arise as a result of such injury or damages.<\/p>\n
